Camper Required Forms
All campers will be required to have the following paperwork on file before they can start camp:
An up to date physical form (Valid for 3 years - Must be dated after 8/30/2019)
2022 COVID-19 Waiver (As of 2/15/2022 this form is still required. If this form is no longer required, we will update you.)Sunscreen Application & Offsite Permission Form
2022 Credit Card Authorization Form (Required for anyone not paying their balance in full)
Parent/Camper Handbook Agreement (Will be available upon publication of 2022 Camp Handbook)
Authorization for the Administration of Medication Forms (If they require daily medication or medication in case of emergencies)
If you do NOT want us to use the Club’s sunscreen in case of emergency (your child forgets theirs or it runs out), you also must complete the Sunscreen Opt-Out Form.
All forms MUST be turned in prior to your child(ren)'s first day of the program. All forms can be emailed to Devon Lawrence, Program Director at
[email protected] You can also fax them to the Club at 203-438-0676. You may also drop off forms in person.
