Camper Required Forms
All campers will be required to have the following paperwork on file before they can start camp:

An up to date physical form (Valid for 3 years - Must be dated after 8/16/2021)

2024 COVID-19 Waiver

Sunscreen Application & Offsite Permission Form

2024 Credit Card Authorization Form (Required for anyone not paying their balance in full)

 2024 Parent Camper Agreement Contract

Authorization for the Administration of Medication Forms (If they require daily medication or medication in case of emergencies)

All forms MUST be turned in prior to your child(ren)'s first day of the program. Forms can be emailed to Fanery Orejuela, Program Coordinator at [email protected]. You can also fax them to the Club at 203-438-0676.  You may also drop off forms in person any time between 10:00am-6:00pm Monday through Friday.
